Form filled in by users, then mailed in PDF

Hi,

I would like to make a form which has to be filled in by a user. Then when he hits send, I would like the server to make a PDF of this input and mail it to an email adres.

Is this possible with a plugin?

They only fill in their name and some other stuff, more has to be already in the PDF, only the user parts are variable.

Can some help me?

Users enter information, and I will post a letter for them… Thats the idea!
