Form Fields to an Existing PDF Template

Hi All,
I have a requirement, I have a predefined PDF file having more than 20 fields in it.
Out of 20 fields, only 9 fields have to be filled from the data received in the form filled via my WordPress website.
Below is an example. In this PDF template, only 9 highlighted fields have to be filled from the WordPress forms.
The form only collects the 9 fields.
How can I generate a final PDF with only 9 fields populated in the PDF?
The rest fields have to be blank.

you can use the “gravity forms” plugin along with the “gravity pdf” plugin to achieve your requirements.
