Form fields not displaying correctly on published WordPress page

    I built a simple form and successfully instantiated a block in the page URL provided. The edit page looked great, but the published page doesn’t display the topic for most of the fields. I tested the page just to see if the back-end was functioning but found it wasn’t. I’ve rechecked that I completed all of the field set-ups and everything looks good to me.
