Form Field Manager

I have 2 different forms on one page. One is a one time donation with multi-level and the other is a recurring donation form with mutli-level. I have added extra form fields for street, city, state, zip and phone. These fields are now showing twice for my recurring donations form. I am using the reveal fields when clicked setting.
Is having 2 forms on the same page causing this problem? Is it possible I have some incorrect settings?
