    I was just asked to look at a site where the forms are all throwing a similar “orange box” error as are being cited in several postings here. I have read and tried several steps that have been recommended for troubleshooting the issue, with no luck so far. Plus, I have an issue with the old developer’s email address showing up in the Reply-To part of the attempted form submission.

    Steps taken so far:

    Updated all plugin and themes.

    I built a new test form with just the basics.

    The JavaScripts are properly loaded in the footer of the source code, so it isn’t the theme blocking JavaScripts.

    I checked the From email address on the forms, and it is a valid email address ending with the same domain name.

    There is no recaptcha on the form, so it isn’t failing there.

    I checked the WP error log file at wpengine.com, and there is no error listed.

    The site was using the Contact Form Advance Database plugin, and no recent entries were shown there, so it is failing prior to adding the form contents to that database. I tried disabling that plugin, but still got the dreaded orange box error.

    The failed submissions ARE showing up in the WP MySQL database. One thing I noticed is that the prior developer’s address showed up in the “Reply To” section of the MySQL entry. There was no “Reply To” address entered in the Additional Headers box. I am still trying to find where that address is stored to be added to a new form created after he was long gone.

    Any help in fixing the failed submission issue as well as finding where this old developer’s address is stored to be used in my new forms from would be most appreciated!

    I replaced some identifying names and site addresses to protect their innocence or guilt as the case may be! ??

    After getting no replies, I dug in and found the solution!

    Delete the plugin and use another forms plugin…
