Form does not show up

Hi:
The plugin looks very nice but I have not been able to get it up and running in my website.
I have installed it and activated it. I created a form but I do not see it in my website. I have followed the instruction video from WordPress but still does not show at the bottom of my posts. Am I doing something wrong? Am I missing a toggle box to activate it?
