Form does not send

I’ve installed the plugin and run it successfully on one site, but on another it’s not letting me submit the contact form – the spinner just keeps spinning and no email goes out. Please help! I also can’t seem to edit the settings once I’ve saved them – there are no options when I select the form I’ve set up the PDF for.
