Form Data not Submitting and emails not being sent

Just updated the plugin today and having a problem. The form data is not being posted to the database and the notification emails are not being sent. Here is a link to the form:

I’ve been getting complaints for awhile now about form data not being received so it may not be just the recent update. Seems to be a recurring problem!
