Flexible Content Issue

I am experiencing an issue when using the Flexible Content add-on. I am not able to create more than 2 Layouts with the Flexible Content field type.
When I attempt to add a 3rd Layout by clicking the “Add New” link, the content of the new 3rd Layout disappears and the data appends to the 2nd Layout.
I am using all unique Field Names and Labels. Here are screenshots:
Any recommendations would be greatly appreciated.
