Fill-in form box and action button are both blank

I’m having the same exact issue. In looking at the site’s HTML it seems that nothing is being passed to the value attribute on the button or the text box. I have not been able to find a solution yet.

Can we ask you to create another popup with the same parameters and check if you face the same issue?
Seems, the popup appears like that because of the absence of one of the fields.
Or you can try to clone the popup and by putting that on the page, check it again.

Make sure to have all the cache cleared before you will check.
On the other hand, you can check via incognito mode as well.

I cloned the popup as requested and tried it in an incognito window. No change.

I’m going to try creating it from scratch, and will let you know if that helps.

I tried re-creating the popup, and I am not having any success.

I created it as type “Subscription” but there is no field to enter an email address and no activate button. As you can see from the attached screenshot, the popup type is “HTML.” The old one is named “New Subscriber” and the new one is named “New Subscribe.”
Also, testing the new one with preview, the background is white, even though I selected a background color of light blue.
