Fill a table from a database

I am building a web site that needs to populate a form from a database. My client doesn’t want an “ecommerce” site, just the ability to generate a form that then gets mailed to them with the order.

All the data will be stored in a database, the form will be filled from the database, and one box left empty, the quantity box.

The form is then emailed to the owner.

I was wondering if there is a plugin that might do what I want. Its so close to a shopping cart, that something like that might work.
