Before running a plugin conflict test we can take a look at the form’s export as well to make sure it’s all well configured. If that’s okay, please share the form’s export:

Hi @indiastartup360,

I imported the form to a test website and replicated the issue. Further checking, I noticed a javascript error in the browser console, which looks related to the Name field added on the second page of the form ({name-2}).

I deleted the mentioned name field and added a new one, resolving the issue on my end. Can you please try it and check if that resolves the issue?
