Do you have an example of what you mean here?

I checked and although its correct when browsing via the media tab it doesnt when accessing through add media to post see screenshot

We only use the default media manager. Does it happen with only YouTube URLs?

I think so yes, the strange thing is if i access videos through the media admin tab they all correctly named but if i access them through add media to the post button it comes up with the file names – strange

That is odd.
