File is not being uploaded to FT

Hi,

I am using the plugin and I am trying to upload a file from a page on my website to a remote folder via ftp. The file is not being uploaded/transferred. In addition, I do not get any notification regarding error or success.

Please find my shortcode below (I changed the username and password of the FTP connection):
