File Error Order completed

Hi Fahad,
thanks for your great plugin, i have the following problem.
When the order status changes to completed. The file attached in the order changes. It’s horrible. You never know which is the right file.

Some Orders have no files attached although the file upload in cart is requiered.
How is it possible? The uploaded file is in the right directory on the server, but there is no file in the order.
