File / Download Manager Plugin

Hello,

I’m looking for a plugin that needs to do the following things. When a user access the page, a subscribe/contact form appears and after filling out that form, appears a download section. I need to know which file the subscriber downloaded.Ex: [email protected] downloaded office.pdf and i want to see this in admin panel/dashboard. If anyone knows such a plugin please tell me..
