Fields showing raw html or nothing at all

I can confirm the same issue and symptoms on my site, running latest GF Version 1.8.19, WP 4.0.

where can I get older version? (3.6.1.2)

You can get old versions of any plugin by clicking the ‘Developers’ tab on any .org plugin page.

try this
open file
plugins => gravity-forms-addons => template-raw.php

and replace
$value = esc_html($value); to $value on line number 209
