Fields not showing

Hello, I have a form, it was working fine before but I believe the most recent update has caused my form to stop working. I have a checkbox as the first conditional infomation, when I select any of the checkboxes, nothing happens! It is supposed to show specific data depending on which box is selected. Here is the form:
