Fields are selected but not showing at checkout or in the order

I’ve created a form in which clients need to select 3 options from a form before they can add their products to the cart. When I tried placing an order to test out the site, I tried to add the item to the cart but I’m getting an error message informing me that the required fields aren’t selected even though they all are. I tried changing the form and making the selection of the options on the form optional as opposed to required fields. I was then able to add the product to the cart but unfortunately, the options that I selected (from the form) aren’t being displayed in my cart or in the order confirmation email that was sent to my admin email address. It’s a simple product that I’m trying to add the form to and I tried updating wordpress, all my plugins, deactivated and then reactivated them and so far, nothing has worked.
