Field Input for Requirement Not Working

Hello:
WP V 4.0
WP-Members V2.9.6I have a field named “5 Digit Location #” and I have it marked as a required field input.
Upon testing the form, no matter what I put in the field, such as a five digit number or letter text, I get this error, “Sorry, 5 Digit Location # is a required field.”
