Featured Images broken, but same full size image isn't

We’re having repeated issues with uploading images. The image used in the story works fine (pulling from cdn.chicagphoenix.com, etc), but for some reason some of the featured images are broken (404 not found) pulling from the exact same location.
Here are a couple of examples:

For some reason, the featured image seems to be uploading to the cdn CNAME reserved for js – I have no idea why.
