Feature Request: Show counter in headers

Great plugin.
One thing which a client has requested is to show the decimal counter number in the header tags also.

e.g. if link in TOC is ‘2.1.3 My subheader’ then the ‘2.1.3’ would be auto prepended to the ‘My subheader’ tag and displayed. For long contents it’s gives the user a better idea of where they are in the document.
