    I can’t log back into my site although the site itself is operating perfectly.

    looks like possible plugin error with formidable
    temporarily deactivate your plugin via file manager or FTP–WordPress dashboard access is not required. rename plugin folder to plugin-old (or something similar)
    this should make it possible for you to access WP-admin and make other adjustments
    do you have access to your webhosting account?
    do you have a recent backup of website files and database?

    Your login address seem to be correct. Is your path for login the right one?
