Fatal Error + Needless Piece of Code

Hello guys!

When I click on “My Posts” this is the only thing I see:

When I click on “Add Post” I see the box to add the post but above the box there is “[wpuf_form id=” ” and below the box there is ” “] ”

How can I fix these two errors?
