FAP Controls on Page Edit Screen Not Showing

I am trying to test out this plugin since Wordbooker is no more. Everything looks good except on the edit page page. The screen options title for the check box is not showing. Also, when it is checked, the area where controls should be on the edit page are blank. I inspected the elements and they have names like fbap1, so I assume these should be the ones showing the controls.

Has anyone else had this issue? How did you resolve it?

I would like to use this plugin but I really need the edit page controls to show up.
