Facebook and Twitter share counter resets to 0 after page refresh

Hello!
I recently installed the Heuman theme for one of my blog and currently I am facing an issue with the share counter of Facebook and Twitter. Google + counter shows fine.

I tried disabling all other plugins in the site and still the same issue. When I share a post, the counter becomes 1. But as soon as the page is refreshed, the count goes back to 0.

Tried clearing browser cache. No luck there too.
