extra fields at top and bottom of form

I installed and set up a form. There are two very short blank fields at the very top before the first entry field (name) and another very small blank one after the CAPTCHA entry field. They cannot be selected and have data entered. I cannot determine why they are there and how to remove them. I would post a link but i am working on localhost. Thanks for any help that can be given.
