Extra field (box) in magic action box?

I cannot seem to remove an extra white box in my magic action box settings. In Mailchimp it is fine and there is only a “Name” and “Email” field but when I add it in wordpress with Magic Action Box I get an extra white box at the end before the button:

Does anyone know why this might be, is it a bug?
