Hi,
If you are using one of the default PDF templates I would suggest first enabling the default-show-empty configuration option (see docs for more details).
Once done, submit a new entry (it doesn’t matter what the data is). Then go to your WP Admin area and edit the newly submitted entry. Delete ALL the field data and save.
Finally regenerate the PDF for that entry. You should get close to what you are after, but may experience issues with radio buttons or select boxes still showing. In those cases I would remove them from the PDF using Adobe Acrobat (they have a free trial so you won’t need to pay upfront for the software).
