Exclude Pages plug-in checkboxes disappear in WP 3.5.1

After upgrading to WP 3.5.1 the exclude pages plugin stopped working. I can’t even see the check boxes that I would select/deselect to include/exclude pages from my site.
Please let me know when this plugin will be compatible with WP 3.5.1 (or now 3.5.2 since that is out now too).
