What does this exactly do?

Hi,

That “No Image Index” tells Google to not index the images on the page. As you want the image to appear as a thumbnail in a regular Google Search you will need to create a disallow rule in your robots file that specifically targets GoogleBot-Image. This rule would tell GoogleBot-Image to not make images appear in a Google Image Search.
