Excellent Plugin & Excellent Support

I tried lots of form plugins and finally settled on Form Maker and then upgraded to the pro version because I needed additional fields over the free version. I found it easy to use and when I did have a question or issue, email support was timely and most helpful. I recommend it and suggest if you need to create a form you should give the free version a try.
