Excel form for requests

  • Hello, i have 1 idea but i dont know how to search it..
    i want to create 1 page in my community for guest, i see my cpanel and i like to do the something like that but different: https://prntscr.com/73ln44

    for example:
    in tample 1: Guest ask something
    in tample 2: my answer
    in tample 3: Green icon for solved
    in tample 4: Red icon if no solved

    do you know who to find something like this or how to create it? thank you and sorry for my english ??
