Everything I wanted and then some! Love It!

  • I spent 2 days fiddling around, trying to make a fillable PDF to upload to my website for clients to choose options. Then I found this – so much easier and it has everything I was looking for: radio buttons, check boxes, text boxes, fieldsets, etc.

    It even has a file upload area. Just tested it and it works great!

    Love this plugin – highly recommend!
