    Howdy Ingo,

    That is pretty mysterious for sure. The plugin will throw an alert dialog box if it gets a failure code, but 200 is not a failure code. The only thing I can think of is that the default version of jQuery is being overridden by either your theme or another plugin.

    What version of WordPress are you using? Can you temporarily deactivate all other plugins and switch to a default theme such as Twenty Twelve and see if the problem persists?

    Yes, changing the theme resolved the issue. Many thanks.

    However, it seems you can’t create a widget – is there a way to manually create a widget? I’d love to see a short survey in the sidebar.

    A widget is on my to-do list for sure but there are challenges to that which I haven’t figured out resolution for yet.
