Error shows up again

does this happen with all other plugins deactivated?

I am having the same issue I cant get past 1st form it just displays the message that form was sent successfully.

I am quite desperate for this to be fixed as I have to push a website live.

I visited the Apply Now link on the homepage. It seems you have a few javascript errors on your page. This plugin depends on javascript. if javascript errors occur on the page, this plugin will not be able to do its job.
