    You need to cover ALL you membership levels with custom forms when you use that addon. So when you edit a user, it can reference the corresponding form for that user’s level.

    You can just create a Generic form which will cover all the levels (when you don’t have a level specific custom form).

    Please create a generic form now from the form builder addon’s interface then you should be good.
