Error messages don’t clear; “disabled” offline donations emails being sent

Hoping you can help with 2 issues! 1) If the donation form is submitted with any errors (missing fields), when these are corrected the error message (give_error div) does not go away and the submit button remains disabled. 2) Although offline donations are disabled in settings, a successful donation triggers (in addition to the appropriate donor receipt email) a second, blank email to donor w/ subject and heading Offline Donation Instructions, AND Admin receives a “pending donation” email w/ heading New Offline Donation w/ instructions to mark as complete w/ the link to do so.
