Error message after recent updte

I updated the plugin this morning and started getting this error:

“Warning: Invalid argument supplied for foreach() in /home/n11nev5/public_html/wp-content/plugins/codemonkeys-hipaa-forms/includes/class-cm-hipaa.php on line 4401”

I turned it off and back on and started getting a different error:

“Warning: Error while sending QUERY packet. PID=2353770 in /home/n11nev5/public_html/wp-includes/wp-db.php on line 2007”
