Error in form donation

Hello.
I have installed your WP Easy Pay plugin free test version, once installed and configured I go to the site and the form and button did not show at all,
to check I opened the developer tool in Firefox, the html code appears but as I told you before, I cant see the form in the page. Console gaves me the following error:
ReferenceError: SqPaymentForm is not defined
