Error em opt-in settings

    when I select SINGLE and then click on UPDATE FORM, I can no longer edit anything, because it seems that all settings lock and can not open and re-edit.
    I can only re-edit when the settings are DOUBLE
    Would anyone know how to tell the form not to lock in the SINGLE option?

    one more detail,
    when I edit the TITLE by clicking on the pencil, I can not save because that “v” to save.
    or I who am not knowing how to save?
