Error After Uploading Video File

So, I just bought Flexi Pro after I was very pleased to finally find a plugin that allowed users to upload files and view their files in a dashboard. However, I can’t seem to upload .mp4 files. I haven’t tried any other file types out yet. I’ve already gone through Mime Type page and selected .mp4. I also made sure to rename the file name to see if that was the problem.

I’ve linked the form on my website (haven’t changed the appearance on the form yet since installing the plugin).
