Entries disappeared after duplicate quiz created!

Been running a quiz for a couple of months now.
Used it to save a template and create a new quiz.
This has removed the entry form and all the entered fields from the original quiz!

How do I restore or find the old entry fields! THIS IS A DISASTER!!!
