Basically, you’ll need to login to FeedBlitz, grab a copy of the HTML code they provide for the form and extract a couple pieces of information.

I’ll add screenshots to the todo list. I’m not a FeedBlitz user, so I’ll add MailChimp ones first and, ideally, community members on FeedBlitz would be able to help fill in any differences in the process.
