  • I found out that I did not add email action on the form I was using. Here are the steps that worked for me:
    – Go to your form that you want to receive email
    – Click on “Emails & Actions”
    – “Add New”
    – “Type:” Email
    – “To:” add your email (the one you want to receive the notification)
    – “Email Message” INSERT ALL FIELDS

    The rest I fill out the way I needed to. Hope it work for you guys.
