Email results button

Hi

I wonder if anyone could help me with a bit of code. I have written a basic calculator as a plugin for my site. There are a set of number fields which the user fills in then when they hit calculate the results are displayed. What I want is a piece of code which will place a button below the results saying “email these results to me”. When the user clicks on it a box appears for them to input their email address and a tick box below it where they can choose to subscribe to my blog. They then get an email with some text and the calculator results.

Any tips/pointers/help would be gratefully received.
