Email reminders

I’m afraid, there isn’t any out of the box for such a workflow on the plugin side.

Mailchimp seems to offer a similar feature when checked:

Maybe you could use the above feature along with Forminator? Thats use the form to submit the data to Mailchimp using the inbuilt integration as mentioned in the following doc:

The rest of the features like setting up specific dates can be configured on the Mailchimp side.

Please do note that I haven’t specifically tested the feature in Mailchimp. It’s more of a suggestion and would recommend you to test it and see if that fits your needs.
