Email Problems

I received and email this morning from my form that said:
From: [your-name] <[your-email]>
Subject: [your-subject]

Message Body:
[your-message]

So after I tested it out even if all of the boxes are filled in it sends the forms this way. How can I fix this? I tried deleting the form and then adding it back and it didn’t work.
