Email Opt-In to get the result

Hi,

I would like to collect the Email adresses from the users. If they finish the quiz, they should enter their e-mail adress, to get the results.

I tried it with the custom fields. But if someone enters their Email adress I get no Emails or a confirmation.

How can I do this?
