Email forwarding

I’m looking for simple plugin that can attach feedback form to the post and email (forward) feedbacks/comments directly to post author’s email. Can be also in form of comment forwarding, but comment not should show on post page.
Something like craiglist replies to ad.
Or like option here “Notify me of follow-up posts via email” – just attached to single post.

If you know any plugin that can do the trick please point me to it.
